The medical records department keeps a complete record of your care and treatment. These records are strictly confidential, and only authorized people have access to your information.
Copies of your health records are provided to you or your doctors upon written request for a reasonable fee. To request copies of your medical records, please write a letter and mail it with a copy of your driver’s license, to the address below. Please include your telephone number so we may call contact you with any questions.
